S-N Counselling Services is committed to providing best practice services to you. This privacy policy outlines my continues requirements to service user.
The Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (the Privacy Act) is adopted. The service user’s data, collection, data release, and storage of these information are in accordance with AASW and the APP’s requirements.
Personal Information
Personal Information is information that identifies an individual. In my practice, I collect basic information: name(s), contact information and date of birth.
Your Personal Information may also be obtained through GP, other health professional or other agencies (paper/electronic) referrals. This information is collected only for the purpose of providing services to you.
Your personal information will be stored in a safe location. Required precautions are taken when there is electronic filing system is in use.
If there is not any contact with the agency over 7 years, your personal information will be destroyed or permanently deleted/deidentify you.
Accessing Your Personal Information
You may want your counselling notes, summary of your sessions to be transferred to another professional. Please contact me by writing.
S-N Counselling Services may request administrative fee for providing the copy of your documents, or fee for written documents.
Disclosure of Personal Information
All information provided by service user is kept secure. Under no circumstances your information will not be released without your signed written permission. However, there are number of exceptional circumstances, which will override your right to confidentiality.
Exceptions are;
- If you become involved in legal matter, inclusive of work matters, the other party may subpoena your file, which is common practice.
- Serious harm to self and/or others
Policy Updates
This Policy may change from time to time due to change in law or as directed by AASW. Any changes will be available on our website.
